In his defense, I think he means that in a bitterly ironic way. I think here he's pointing an accusing finger at the listener for his lack of compassion for the starving, suffering multitudes on that benighted continent where there is no food or water and in fact, no snow even, and where -- get this -- They don't even know it's Christmas!

But maybe not! The rest of the song is so awful, maybe he is completely serious.

I was always confused by "do they know it's Christmas any way". As in, why should this matter? Until I realized that what he meant was that they were benighted nonwhite nonwestern non-Christian kiddies who were missing out on the perfect neo-Dickens Christmas that all us WASPs take for granted and it was our obligation as wealthy Christians to BRING IT TO THEM.
